Please complete all of the questions below; they are critical in booking your flight. The flight will be booked within a few days of completion of this form. Nikki Chase, AP-TIP Program Coordinator, will contact you prior to booking your flight to ensure that the flight arrangements are as close to your preference as possible and are reasonably priced.

A rental car, if necessary, (Enterprise Car Rental) will be also be booked by the University. The confirmation information will be sent to you once the flight has been booked and will be included in your itinerary.

Once your flight has been booked, you may NOT change the date/time/location of the flight without approval from Nikki. You can however, add your frequent flyer number.
